The child must be a North Dakota resident, residing in state, and in custody of a human service zone.

 

Eligibility Criteria:

1. The child must be identified as being involved in multiple systems, such as Juvenile Justice (Dual Status Youth), Developmental Disability programming, Behavioral Health (HSC/skills training), and/or Protection and Advocacy.

   AND one of the following:

2. Three placement disruptions in a six-month time frame

3. Child is residing in an unapproved placement setting (ie: office/hotel)

4. Inability to achieve placement stability at their identified appropriate level of care and/or no current discharge plan from a higher level of care.

 

The CCC Program does not include Tribal Nation (IVE or 638), Division of Juvenile Service or 18+ Continued Foster care cases.
